WILMINGTON — The following information is obtained from the Clinton County Health Department and is compiled from inspection reports.

Violations are either critical or non-critical. Critical violations are more likely to contribute to food contamination, illness or an environmental health hazard, including inadequate cooking of food items or poor personal hygiene.

Trula’s General Store, 384 W. State Route 350, Wilmington, 6-18-15, standard. No violations noted.

Blanchester Veterans’ Memorial Park, 933 State Route 1235, Blanchester, 6-23-15, standard. No violations noted.

Streber’s Market, 299 S. South St., New Vienna, 6-25-15, standard. Boxes of heads of cabbage stored under meat cuber table in walk-in cooler. Carton of eggs stored on shelf above Streber spread. Streber spread and coleslaw missing date markings. Several items missing ingredient info. Can of Similac soy formula missing label. Wall peeling in walk-in cooler in deli. Lid missing on trash can in women’s restroom. Lids missing on outside dumpsters. Wall damaged near faucet of mop sink.

New Vienna 1st Stop, 101 N. South St., New Vienna, 6-25-15, standard. Labels missing on sausage gravy for customer self-service. Ingredient info missing for bulk, self-service donuts. Light not working in holding cabinet.

Family Dollar Store, 105 E. Main St., Clarksville, 6-23-15, standard. No violations noted.

The Corner Market, 10 Main St., Clarksville, 6-23-15, standard. Tea not in labeled container; corrected. Boxes of light bulbs above Pringles canisters on retail shelf; corrected. Sliced ham found dated June 12 and bologna dated June 15; items discarded. Egg patty wasn’t kept cold enough. Ice on the back of and water in the bottom of the cooling unit. Thermometer missing in pizza hot holding cabinet. Debris/residues noted on floor of walk-in cooler near milk storage. Black residues noted on dispensers of Icee machines. Cutting boards on prep coolers have cutting/staining marks. Incense ashes found near pizza cut table and shake mixer; corrected. Roller brushes found on work table.

Ollie’s Bargain Outlet, 1633 Rombach Ave., Wilmington, 6-24-15, 30 day inspection. Dumpster missing lids. Restrooms missing hand washing signs.

Wilmington Child Care, 1600 Alex Drive, Wilmington, 6-24-15, standard. Thermometer missing in refrigerator.

Gold Star Chili, 1295 Rombach Ave., Wilmington, 6-24-15, standard. Edge of counter separating near ice bin. Side of counter unit separating/deteriorating next to steam table. Shelving near bussing tubs deteriorating; new cabinets on order. Warming plate has food debris. Shelving in walk-in cooler has food residues. Shredder equipment had food debris accumulations.

Taco Bell, 1701 Rombach Ave., Wilmington, 6-24-15, standard. Brown/black residues noted on soda fountain of drive-thru.

Brown’s Distributors, 100 E. Main St., Martinsville, 6-26-15, standard. Bags, boxes and cans of food stored on floor. Turkey pizza toppings missing date marks. Packages of raw sausage and bacon stored above a gallon of sweet tea and bagels. Interior surfaces of walk-in cooler are not smooth, non-absorbent and easy to clean. Splatter and staining on wall behind hot plate equipment. Cloth used with dish strainer. Cloth found on hand sink. Space seen at exterior exit door in walk-in cooler. Floor tile damaged in front of pizza make line cooler.

Village Quick Shop, 50 Main St., Martinsville, 6-26-15, standard. Door seal damaged/loose on pizza prep cooler. Lights not working in doors of walk-in cooler. Food residues observed on slicer. Black residues observed on ice dispenser on soda fountain machine. Dust accumulated on fan covers on cooling units in walk-in cooler. Floor tile damaged, and ceiling lights not working.
