Restaurant inspections: Long list for one

WILMINGTON — The following information is obtained from the Clinton County Health Department and is compiled from inspection reports.

Violations are either critical or non-critical. Critical violations are more likely to contribute to food contamination, illness or an environmental health hazard, including inadequate cooking of food items or poor personal hygiene.

The following restaurants or food service establishments were inspected recently and violations/comments include:

• Wilmington Dairy Queen, 1093 Rombach Ave., Wilmington, May 16. Critical: Employee making sandwiches with exposed burn on her arms. All employees need to wash hands with soap and water as often as necessary to ensure hands are clean. All employees who have cut/burn must bandage and glove/cover area. There was an ice scoop down in the ice machine. The scoops in the sprinkles and peanut butter crunch were down in the product. The large chunk of cheese in reach-in prep cooler was not properly labeled or dated. The bacon that is held by time in lieu of temperature was not properly labeled/dated. The sliced cheese on the prep cooler was at 49°F. The beater on the mixer was broken. The icing decorating tips were dirty. The knife on the counter had food residue. The cake-decorating pedestal was dirty with icing debris. Inside of ice machine was dirty. There were two ice cream cakes in the display case that did not have ingredients listed.

There was condensate dripping from walk-in freezer. The threshold of walk-in freezer was pulling apart and had dirt/debris. The mixer was dirty. The counter by the deep fryer had large build-up of grease. Seal of reach-in cooler had food debris in it. The outside of the microwave was dirty. The front of warmer with drawers was dirty. The pop machine had splatter on ice chute. The dumpsters outside had the lids open. There was trash on the floor of walk-in cooler. There were mop heads and brooms stored in mop sink. There was unused equipment on floor of dry storage area. There was trash on the floor of dry storage area. There were chicken strips on the floor by the deep fryer. The wall was dirty by pop machine in drive-thru area. There was trash on the floor in drive-thru area. There were boxes stored on floor of walk-in freezer.

• Domino’s Pizza, 265 W. Locust St., Wilmington, May 12. Written policy for clean-up of vomit/diarrhea incidents unavailable. Verification employees have been informed when to report illness is unavailable. Level 2 training/food protection certification unavailable. Trash receptacle unavailable at handwash station. Handwashing sign missing at hand sink. Ceiling light fixtures nonworking in mop sink room and near Coke reach-in cooler. Wall and cove trim is not sealed near hand sink (open gap in materials). Mop was stored on floor near hand sink. Expired license is posted. Facility remodeled portion of facility without proper review and approval.

• Angilo’s Pizza, 142 S. Broadway St., Blanchester, May 12. Follow-up. Violations corrected: 3.2, 4.0, 4.1, 5.1, 5.4, 6.1, 6.2, 5.4, 6.4, 7.0. Level 1 and Level 2 certifications unavailable. Critical: Prep cooler upright: 43°F, BBQ sauce 43°F, cottage cheese 40°F, stainless door upright 45°F, spaghetti sauce 43°F. Salad prep cooler: 42°F, sliced turkey 50V, pickles 50°F, chicken salad 47°F. Equipment holding TCS foods not able to maintain safe temperatures.

Dust observed on vent hood above fryers. Ansul tag/fire safety equipment has date of 2015. Sprayer at 3 sink is hanging down in the compartment of the sink. Prep cooler upright is leaking water when the door is opened.

Follow-up: May 19

• Domino’s Pizza, 211 S. Broadway St., Blanchester, May 12. Verification employees have been informed of illness reporting requirements unavailable. Level 2/food protection certification unavailable. Critical: Sliced ham 44°F, pepperoni 43°F, sliced sausage 43°F (pizza make line).

Sticker residues observed in dishes being sanitized. Storage shelf with pizza pans was not 6 inches above floor. At time of visit, drive-thru window was open. Trash can with lid is unavailable in women’s restroom. Handwash sign missing as hand sink. Corner trim missing on wall covering corner across from 3 sink. Shield (metal) on condenser unit in walk-in cooler is not secure.
