Clinton County eateries inspected

WILMINGTON — The following information is obtained from the Clinton County Health Department and is compiled from inspection reports.

Violations are either critical or non-critical. Critical violations are more likely to contribute to food contamination, illness or an environmental health hazard, including inadequate cooking of food items or poor personal hygiene.

The following restaurants or food service establishments were inspected recently and violations/comments include:

• Engine House Pizza, 134 Main St., New Vienna, Aug. 11. Verification demonstrating employees have been informed when to report illness unavailable. Residential non-NSF equipment in facility. Air dry rack at 3-sink has coating peeling/rust accumulations. Water in bottom of pizza prep make line. Ice accumulated in bottom of Frigidaire upright freezer. Seal is damaged on unit. Seal is damaged on side-by-side refrigeration unit. Equipment holding bags of cheese not maintaining correct temperature. Observed black residues on ice machine inner guard. Observed clean bowls on cart stored on cardboard. Light is not available at soda bib storage area. Light fixture in kitchen nonworking. Walls throughout kitchen have chipped paint. Ceiling has water damage and/or staining throughout kitchen. Floor has chipped paint and/or need floor sealed. Soda bib area needs cleaning to remove insulation debris, etc.

Critical: Cooler with shredded bag cheese had air temperatures in mid 50sF. Tarter sauce dated use by Aug. 5. Air gap not visible at soda fountain machine drain. Mouse droppings observed in cabinet below soda fountain machine.

Follow-up: Aug. 25.

• Continental Manor Nursing & Rehab, 820 E. Center St., Blanchester, Aug. 4. Wet cloth stored on cart. Irreversible thermometer measured 150°F and 152°F (must measure 160°F or above). Light intensity low in walk-in freezer and cooler. Windows had cobwebs and other debris accumulated. Ice (minor) and debris accumulated in floor in walk-in freezer. Wet mop observed under prep sink. Observed dripping water. Floor tile damaged under prep sink. Floor below/near warewash machine damaged/dirty; needs resealed/painted?

• Cowan Lake Commissary, 1756 Osborn Road, Wilmington, Aug. 9. Food handler hair was not restrained. Heavy ice accumulated in unit and on product in MaxxCold reach-in freezer. Ice built up on shelving in upright Holiday freezer. Food splatter observed in microwave. To-go food containers stored on bottom shelf of prep table in kitchen and is less than 6 inches from floor. Sanitary trash can with lid missing in restroom. Observed cobwebs, bugs, etc. on walls in stock room. Threshold missing between floor of dining and storage/stock room. Duct tape is not a durable repair. Cove trim missing next to Hershey freezer in kitchen.

Critical: Date marks missing on sausage gravy, hot dogs, sliced tomatoes, sausage, pepperoni, etc.

• Brown’s Distribution, 100 Main St., Martinsville, Aug. 11. Level 2 certification in food protection unavailable. Verification demonstrating employees have been informed when to report illness unavailable. Food handler hair not restrained Sleeves on pizza boxes stored on floor. Food debris observed in reach-in equipment (coolers/freezers) and slicer. Dust/dirt/debris observed in walk-in cooler. Unfinished wood/pressboard in walk-in cooler. Light bulbs nonworking in kitchen ceiling fixtures. Wet mop was stored in kitchen.

• Holiday Inn Express, 155 Holiday Drive, Wilmington, Aug. 10. Remodel is complete. Raisins and other food container stored under hand sink. Salt packets stored under 3-compartment sink.

• Wilmington Nursing & Rehab, 75 Hale St., Wilmington, Aug. 10. Everything currently corrected.

• Arby’s, 1619 Rombach Ave., Wilmington, Aug. 10. Complaint. Received complaint regarding employee having long hair and it getting into customer’s food. Employee has long hair in ponytail that hangs down back. Hair must be properly restrained with hat, hair net, braid or bun. Back door of facility propped open. All doors to exterior must be screened or closed to prevent insects.

• Spillway Lodge, 623 Old State Road, Clarksville, Aug. 9. Critical: In the M3 turbo refrigerator several items were not labeled/dated (cole slaw, potato salad, mushrooms, onions, cheese, etc.). Warewash machine thermometer was only reading 100°F (must be 120°F).

Counter by can opener is coming off. Counter by coffee pot is missing laminate. Counter/laminate by hand sink in service area is coming up. Air conditioner missing front cover. Microwaves have paint peeling and cracked plastic (all 3 over stoves).

Follow-up: Sept. 13.

• Wilmington Middle School, 275 Thorne Ave., Wilmington, Aug. 9. Walk-in freezer door is still icing up, light fixtures still have ice/water in them. Employee had to chip ice off door and door seal today to get door to close.

• Cluxton’s Dairy Bar, 267 W. Main St., New Vienna, Aug. 11. Floor paint damaged/chipped near soft-serve equipment and bathroom.

Critical: Cole slaw had date of July 23.

• Frisch’s Big Boy, 1341 Rombach Ave., Wilmington, Aug. 11. Follow-up. Violations corrected: 2.2, 3.2, 3.4, 4.1, 4.4, 6.4. Thanks for cooperation! Light missing in salad bar unit.

• Autumn Years Nursing Center, 580 E. Washington St., Sabina, Aug. 14. Violations corrected: 2.3, 2.4, 3.4, 4.1, 4.4, 5.4, 6.2, 6.4. The following require correction: Critical: Ice machine was relocated from break room to near exit of kitchen. Air gap is not visible on ice machine (drain). Is there a backflow prevention device on water supply line? Ice machine relocation must be completed/inspected under permit and performed by a licensed registered plumber.

Follow-up: Aug. 28.

Critical: Bologna, sausage gravy, taco meat, sliced chicken, lettuce, etc. missing date marks. Half & Half stored on counter at coffee station.
