Walk-in cooler shelving food debris accumulations. Hand sink missing soap and paper towels. Ash tray, cigarette pack and lighter found on storage rack with clean dishes and food supplies. Floor damaged near hot water heater, dish washing area and under cooking equipment. Wall damaged in women’s restroom. Grease and food debris accumulations observed on floor under/near broaster, fryer, grill, stove, oven and storage shelves. Mouse droppings observed under rack.

Ingredient info missing for bulk donuts. Donuts must be covered or under a lid. Light not working in walk-in cooler. Pepto bismol found in box with seasoning. Toilet in restroom leaking. Floor paint worn in cutting areas.
